At St. Luke's Home Care, we provide a variety of in-home health care to children and young adults with serious medical conditions. Our goal is to provide the best care to our patients, while making their parents feel comfortable and confident in caring for their children at home. Our family-centered care philosophy draws on the support and involvement of the entire family and we work hand-in-hand with primary and specialty care providers.
St. Luke's Home Care provides the following:
- Registered Nurses and Licensed Practical Nurses experienced in providing care according to the patients' individual needs and the doctor's orders. This includes both extended stay and skilled visits.
- The ability to care for complex pediatric clients in the comfort of their own home, including ventilators, IV and TPN needs, trachs, g-tubes, central lines, respiratory medication, and monitoring equipment of all types.
- Pediatric therapies such as physical therapy, occupational therapy, speech therapy and respiratory therapy.
- Home care aides for basic respite care and to assist with the activities of daily living.
- Individualized assessment of each client and family's needs.Home medical equipment such as ventilators, wheelchairs, nebulizers and others.
We participate with a variety of payer sources such as Medicaid, waiver programs, EPSDT and private insurance.
