Benefits Summary

Full-time employment is 64 to 80 hours per pay period (two weeks).

Part-time employment is a minimum of 32 hours per pay period (two weeks).

Group Medical Plan

Two comprehensive health insurance plans with coverage for medical, mental health and prescription coverage.

The two options for medical include IHS PPO and IHS High Deductible. You may choose from Employee and/or Employee plus one dependent and family coverage.

Prescription coverage through participating pharmacies. Co-payment for prescriptions are determined by: Generic: $10; Preferred Brand: $20 and Non-Preferred Brand: $35.

Group Dental Plan

One comprehensive dental insurance plan is offered.  The plan is Delta Dental with orthodontics.  Orthodontics for adults and/or cosmetic procedures are not covered under this plan.

Term Life Insurance

Core life for a regular full-time employee is 1 x base annual salary rounded up to nearest $1,000.

Core life for a regular part-time employee is $10,000.

Eligible employees are given the option to carry additional life insurance on themselves, spouses and their dependents.

Short Term Disability

Income protection for eligible employees unable to work for health reasons. Pays 60% of weekly earnings from the 14th day through six months with medical certification.

Long Term Disability

Income protection of 60% of monthly salary provided for total and permanent disability following a six-month waiting period.

Paid Time Off (PTO)

PTO combines vacation, holidays, sick and personal days into a flexible plan with a maximum to up to 200 hours (25 days) per year for employees with fewer than five years service. Accrual rates increase after five years of employment is completed.

After six months of employment; for vacation and illness.

401(k)

Pre-tax retirement savings plan. IHHC will automatically contribute 2% of your pay into your account each year. You may contribute up to $15,000 annually and Iowa Health Home Care will match 50% of your contribution up to 6% of your pay.

Health Care Spending Account (Flex Account)

Optional payroll deduction program for a pre-tax salary reduction account for qualifying out-of-pocket dental and medical expenses. Maximum contribution per year is $5,000.
